Here is a list of forms that have been found on the internet for use:

    Please remember that if your school district is not listed here does NOT mean that you cannot use an exemption.  If your district is not listed then use the generic form that I created and have used in the past. Here is another one that basically says the same thing that mine says, but since it's in a form format and doesn't say whether it's religious or philosophical it would probably be more easily accepted by the schools, etc.

Ohio Department of Jobs and Family Services - I correct my previous assumption of what the purpose of this form was.  Quoted from the bottom of the form - "This is an optional form provided by JFS which may be used by child care centers and type A homes to meet the requirements of Rules 5101:2-12-37 and 5101:2-13-37."  FYI - a Type A Home is an in-home daycare where the licensed owner of the daycare has a contract with the county JFS to provide daycare.

Public school districts:

• Defiance City Schools - One page form, lists all forms of exemption, although it asks which exemption and it asks for a reason.
• Delaware City Schools - One page form, does not ask which particular exemption you are choosing and it does not ask your reason.
• Perrysburg School District - One page form, this form makes it look like there are only two options for exemption - medical and religious.  It does ask which exemption you are asking for but does not ask your reason.

Universities or colleges:

• Case Western Reserve University - Students, researchers, professors - This is a bloodborne pathogen survey, it has a statement about not wanting the Hep B vaccine before using human blood and tissue in their teaching and/or research, but this is not the exemption itself.  There is a phone number listed to call University Health Services, they may have the exemption form.  I would just try writing a letter first.
• Columbus State Community College - This file is a health statement, the exemption for the Hep B shot is on the third page.  At this time I don't know what you would have to do to be considered exempt from the rest of the vaccines.

Employers:

• Twinsburg City - Hep B Vaccination Informed Consent and Blood Borne Pathogen Training, exemption is on page 2.

Another idea, you may want to keep a copy of your exemption on file at your children's doctor's offices just in case.
